Bethany Housing tenants may use this form to report and submit an income change. We will need four weeks' worth of paystubs (if work-related) to process this request. Documentation for child support change will need to be sent along with the form.  Accepted file formats are jpg, png, doc, docx, and pdf. The maximum file size is 4 MB.
